HIPAA Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED, HOW YOU CAN GET ACCESS TO THIS INFORMATION, AND YOUR RIGHTS. PLEASE REVIEW IT CAREFULLY.

RCC’s duties

RCC is required by law to maintain the privacy and security of protected health information, provide this Notice, follow the Notice currently in effect, and notify affected individuals following a breach of unsecured protected health information when notification is required. RCC will not retaliate against you for exercising a privacy right or filing a complaint.

Uses and disclosures for treatment, payment, and operations

• Treatment: RCC may use or share information to provide, coordinate, or manage your care, for example with your treating clinician, laboratory, or pharmacy.
• Payment: RCC may use or share information to bill you, process authorized payments, and perform payment-related administration. RCC is primarily cash-pay but still performs payment functions.
• Health care operations: RCC may use or share information for credentialing, quality assessment, training, compliance, auditing, business planning, security, and other lawful operations.

Other uses and disclosures permitted or required by law

Subject to applicable conditions and minimum-necessary rules, RCC may use or disclose information for public-health and safety activities; reporting abuse, neglect, or domestic violence; health oversight; judicial or administrative proceedings; certain law-enforcement purposes; coroners, medical examiners, and funeral directors; organ and tissue donation; workers’ compensation; research when legally permitted; averting a serious threat to health or safety; specialized government functions; and other disclosures required by law.

Your written authorization

Uses or disclosures not otherwise permitted by law generally require your written authorization. This includes most uses of psychotherapy notes, most marketing communications involving payment, and a sale of protected health information, if applicable. You may revoke an authorization in writing, except to the extent RCC has already acted in reliance on it.

Your rights

• Inspect and obtain a copy of designated medical and billing records, subject to limited legal exceptions. RCC may charge only a lawful, cost-based fee.
• Ask RCC to correct or amend information you believe is incorrect or incomplete. RCC may deny the request in writing for a lawful reason.
• Request an accounting of certain disclosures made during the applicable period.
• Request a restriction on certain uses or disclosures. RCC is not always required to agree, except where HIPAA requires a restriction for a disclosure to a health plan concerning an item or service paid in full out of pocket.
• Request confidential communications by a reasonable alternative method or at a reasonable alternative location.
• Receive a paper copy of this Notice at any time, even if you agreed to receive it electronically.
• Choose someone with legal authority to act for you. RCC will verify that authority before taking action.

Effective date and changes

Effective date: August 4, 2026. RCC may change this Notice and make the revised Notice effective for information already held and information received later. The current Notice will be available at the office and on RCC’s website.
