Subscription Billing Agreement

How recurring subscription billing works, what’s included, and how cancellation affects charges.

A subscription is not insurance and is not a promise of any diagnosis, clinical outcome, prescription, refill, medication, or pharmacy service. The specific plan, recurring amount, billing cadence, and start date are completed and signed on your written plan schedule during intake.

Recurring payment authorization

By accepting a plan, the patient authorizes RCC and its payment processor to charge the payment method on file on the selected billing date until cancellation becomes effective. The patient will keep payment information current. RCC does not intend to store the full card number in the medical record.

Included and excluded items

Only items expressly listed in the selected written plan schedule are included. Unless that schedule states otherwise, outside laboratory charges, consultations, imaging, urgent or emergency care, specialist care, pharmacy shipping, and services from unaffiliated providers are excluded. Medication cost may be included only when the selected plan expressly says so and remains subject to clinical authorization and pharmacy availability.

Failed payments and administrative holds

RCC may retry a declined payment, contact the patient, assess a disclosed administrative fee, or place an account on administrative hold in accordance with the patient’s current signed plan terms and applicable law. RCC may waive a fee or hold when appropriate. Any hold will be managed consistently with clinical safety, continuity obligations, and Provider judgment.

Changes and disputes

RCC will provide advance notice of a material price or plan change before the first affected charge. The patient should promptly report suspected duplicate or incorrect charges. A billing dispute does not authorize a chargeback for services knowingly received, but it does not waive any lawful dispute right.

Cancellation

Cancellation is governed by the Cancellation and Refund Policy. Cancellation stops future recurring charges after its effective date; it does not erase valid charges incurred before that date.
