Patient Complaint Procedure
How to raise a concern with RCC, and how we handle it.

RCC welcomes concerns
A patient may raise a clinical, billing, privacy, access, service, pharmacy-coordination, or discrimination concern without retaliation or adverse treatment. A complaint does not replace emergency care or an appeal or reporting right provided by law.
How to submit
Contact RCC by secure patient message, telephone at (386) 946-6375, or written mail to 2541 S. Volusia Ave, Suite 100, Orange City, FL 32763. Include the patient’s name, dates, people involved, what occurred, desired resolution, and supporting material. Avoid sending sensitive medical information by ordinary email.
RCC process
- RCC will log the complaint, protect it as appropriate, and acknowledge it generally within two business days.
- The matter will be assigned to a person who was not the sole subject of the complaint when reasonably possible. Clinical issues go to the Provider; privacy issues go to the Privacy Contact; billing issues go to administration.
- RCC will preserve relevant records, investigate proportionately, request additional information if needed, document findings and corrective action, and communicate an outcome or status update.
- No fixed outcome is promised. Complex matters, third-party records, or legal obligations may require additional time.
External complaints
Patients may file a health-information privacy complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, at https://www.hhs.gov/hipaa/filing-a-complaint. A complaint about a Florida-licensed health-care practitioner may be submitted through the Florida Health Care Complaint Portal at https://complaint-portal.mqa.flhealthsource.gov. Pharmacy dispensing or product concerns may also be directed to the dispensing pharmacy and the appropriate pharmacy regulator. RCC will not retaliate for a good-faith report.
Urgent safety issues
Potential emergencies must follow the Emergency and After-Hours Notice. Suspected medication contamination, labeling error, serious adverse event, or recall concern should be reported promptly to both RCC and the dispensing pharmacy.
